Got Transition, National Family Advisory Group y con amplios aportes de un grupo de trabajo de padres líderes de habla hispana pertenecientes a family voices, han desarrollado un conjunto de herramientas para que las familias lo utilicen para guiar la transición de sus jóvenes de la atención médica pediátrica a la de adultos.

El kit de herramientas incluye recursos fáciles de usar para jóvenes y familias para ayudar a los jóvenes a asumir una mayor independencia en el cuidado de su propia salud y el uso de los servicios de salud totalmente en español. Los materiales incluyen una línea de tiempo de transición, preguntas para hacerle a su médico, lo que significa cumplir 18 años en el cuidado de salud y más.

Got Transition and its National Family Advisory Group, with extensive input from a Family Voices workgroup of Spanish-speaking parent leaders, have developed a toolkit for families to use to guide their youth’s transition from pediatric to adult health care.

The tool kit includes easy-to-use resources in Spanish for youth and families to help youth assume more independence in taking care of their own health and using health services. Materials include a transition timeline, questions to ask your doctor, what turning 18 means for one’s health, and more!
